Suwidha Centre Jalandhar has started accepting the Applications of Passport from 1st April 2008.The Services are available at Counter No 20,21,22. Presently only fresh application for New Passport are being accepted at the Counter. For submitting the application, Applicant hs to come personally with the Original Documents, Dully copleted Forms & Application Fee for the Passport Office in shape of Demand Draft.
Please Note that Suwidha Centre Jalandhar doesnot accept Applications for Minor Cases. Detail Process, Required Documents,Fee & other information is described below :-

Documents Required

1.   Passport Application Form No1:

  Along with 3 Photograph & Signature of Applicant. Signature doesn't Mentioned on Front Photograph instead of P.P. Formís photo.

2.  Date Of Birth Proof:

Birth Certificate, School Certificate issued from Board (i.e., 5th, 8th or 10 th or last school attended by the applicant) Character Certificate issued from School won't be acceptable, Affidavit as per Annexure 'A' by illiterate applicant. If applicant born on or after 26- 01-89 only Birth Certificate issued by the Municipal Authority or office of the Registrar of Birth & Death is acceptable. It has been issued from Registrar neither from 'Chowkidar' because he is labourer. If applicant's name does not enter on birth Certificate than we can also receive that. Date of birth Certificate, which has been issued from Sarpanch, wonít be acceptable. So, We will have to receive that Birth Certificate which has been issued from Gram Panchayat. Late Entry would be accepted instead of 'Not found'. If applicant is Ex-Serviceman and he does not have sufficient proof of his Date of Birth than he can submit his Discharge book as a proof for Date of Birth along with his Residential.


3.   Residence Proof

Ration Card, Electricity / Water Supply /Telephone bill (on behalf of Parent's Name), Statement of Running Bank Account, Voter ID Card. If applicant submits only Ration Card than we need one more proof of Address. If applicant attached Voter ID Card than he won't be liable to produce another Residential evidence. Driving License won't be accepted. If applicant has across the age of minority (such as 18 years) than he has to submit Ration Card as a residential proof and he has to liable for submission of his parentís Voter ID Card or Electricity/Telephone/Water supply bill. Residential Certificate issued from Tehsildar won't be accepted. If Surname of applicant's father has been mentioned on School Certificate but not as on Residential evidence,even he fill the form along with the Surname of his father than he does not need a proof of that e.g., Father Name of applicant is Rana Partap Khurana which has already been mentioned on School Certificate but Residential proof shows only 'Rana Partap'. Even applicant filled a form along with the surname of his father than we can accept his file. The name of applicant, father's, mother's name would be taken as per School Certificate instead of Residential evidence. In other case, if Surname of applicant's father has not been mentioned on School Certificate but already mentioned on Residential evidence, even he fill the form along with the Surname of his father e.g., Father Name of applicant is 'Rana Partap' which has already been mentioned on School Certificate but Residential proof shows 'Rana Partap Khurana'. Even applicant filled a form along with the surname of his father than he needs to be given a Deed poll/Sworn affidavit as per Annexure 'E' along with the cutting of two local leading News Papers. If an applicant has been shifted from other District before 8 months and he does not have a proof for 1 year residency than he has to given extra 2 P.P. Forms along with the application. If, applicant has been residing less than a year and he want to make a Passport on that current Address then he will have to mention his old Address in Column No. 10.


4.   Annexure 1

    Annexure 1 in shape of Affidavit (Along with Date of Birth & Place of Birth). If this has been attested from Notary, there should be a witness detail upon it and mentioned a Serial No. along with date from Notaryís Register.

Fresh Passport After Marriage


    If a woman is appliying for a fresh Passport after marriage,than she will be liable to produce the following documents: -

1.   Passport Application Form No. 1

   Along with 3 Photograph & Signature of Applicant. Signature doesn't mentioned on Front Photograph.

2. Date Of Birth Proof

2.Residence Proof

        Ration Card, Electricity / Water Supply /Telephone bill(on behalf of Parent's Name), Statement of Running Bank Account, Voter ID Card. If applicant submits only Ration Card than we need one more proof of Address from above categories. Photo copy of the husband's Passport, if any.

NOTE : If an applicant has been applying her passport within 1 year from the date of her Marriage whether the name of Spouse has been changed or not. Then, She will have to Produce either Marriage Certificate or Joint Affidavit as per Annexure 'D'. Then her name will be entered in Ration Card also. If Parents of Spouse are citizen of other District than She have to Produce the extra 2 P.P (Personal Particular) Form. If applicant has been applying her Passport after 1 year of her marriage than we neither need Marriage Certificate nor Joint Affidavit. If applicant has been applying Passport after marriage and her name changed due to her marriage than she will produce a deed pool affidavit as per Annexure ĎEí along with the 2 copies of Local leading Newspaper.


4.   Annexure 1

    Annexure 1 in shape of Affidavit (Along with Date of Birth & Place of Birth). If this has been attested from Notary, there should be a witness detail upon it and mentioned a Serial No. along with date from Notaryís Register.

Verification of Fresh Passport:

        Personal Particular Form (P.P.) form will be stapled. Receipt number would be mentioned on P.P. form. We will be taken a two copies of print out relates with concerned day's receiving and enter the Challan Number on that list. We will wrap those cases in a Pack and sealed that along with the Statement. So, that we will hand over that to SSP Office. Whenever SSP office will issue those P.P. forms after making Police Verification (entitled with CID Intelligence, Chandigarh & local Police Verification) so that we have to compare these P.P. forms along with Challan issued from SSP Office. NOTE :- If SSP Office recommended the verification of concerned applicant than we have to be entered report in Computer even if SSP does not recommended Police Verification of concerned applicant than means the verification of concerned applicant has not be cleared. We will write a note ' Police Verification does not recommended /cleared from SSP office and dispatch that file to Passport Office.

        Signature of issuing authority of draft will be mentioned at the right hand corner side of the draft and issuing branch will be display at the upper side of left corner side of draft. It neither be cut. It should be Jalandhar Branch at the end of Left side of the Front of Draft whether the draft has been issued from Nakodar, Phillaur, Amritsar etc but payable at Jalandhar Branch neither Nakodar, Philluar Nor Jamsher, Phagwara etc. The validity of Draft is upto 6 months but we have to accept the draft is to be within 2 months from the date of Issue. Application number will be written on backside of draft such as Jalandhar 'Z'.

Receipt

       Computer generates 2 copies of receipts. One is to be given to applicant, second is to be office copy attach with the Passport form along with the draft in the middle page of Passport Application main Form (Draft will be upper from of Receipt and both will be staple along with main Form). Application number is to be generating from Computer i.e., 'Jalandhar Z' series behind the token number. Application number should be written by us at the behind side of snap of P.P. Form. Both P.P. Forms will be forwarded to S.S.P Office (one is to for SSP office & second is to for C.I.D. Chandigarh but we both P.P. forms forwarded to SSP office, Jalandhar.

        Suwidha Charges related to the submission of application of new Passport is just 100 Rs. Only and No Government Fee has to be paid by applicant. Applicant has to submit draft of 1000 Rs. from the above mentioned banks.
